LIAISON REPRESENTATIVE, per diem
GENERAL SUMMARY/ OVERVIEW STATEMENT:
The Liaison Representative is responsible for performing preadmission patient evaluations and participation in referral relation activities. Most Liaison Representatives are assigned to one or more referring hospitals as their primary responsibility.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Evaluate and interview patients (and/or their family member or significant other) referred to PCC and SRHN for admission.
2. Determine rehabilitation potential and level of care of patients evaluated. Make appropriate decision regarding acceptance to SRHN. Communicate and consult with SRHN staff as needed when making the decision to accept or reject.
3. Communicate decision to referral source; discuss options if patient is not deemed appropriate for admission to SRHN.
4. Assist referring facilities with difficult transfers. Coordinate difficult transfers with bed manager and Intake
Coordinator. Educate referring physicians and Case Managers regarding capabilities of SRHN and PCC
Facilities.
5. Educate referral sources on an ongoing basis regarding rehab level of care criteria and new programs.
6. Work closely with the Program and Medical Directors in coordinating in-services, educational offerings, and marketing events to primary referral hospitals.
7. Inform the Clinical Manager and Director of Admissions of any issues, trends or needs of primary referral sources at weekly summaries and verbally as needed.
8. Execute pilot programs as agreed upon by Admission leadership.
9. Performs other duties as assigned by the Clinical Manager, Directory of Admissions.
QUALIFICATIONS:
Registered Nurse, PT, OT, SLP, Social Worker with a minimum of 3 years of clinical experience. Experience in acute rehabilitation is preferred. Knowledge regarding levels of care, insurance regulations preferred. Sales and marketing experience desired. Excellent interpersonal and communication skills. Ability to meet and interact with new people. Possesses good interviewing techniques
